Phase Hockey programs can be registered for right here on the website. Click Register Now and fill out your information and submit it. Mail your payment in and your spot is held. All your information is kept private. If you wish to fill out an application form by hand and send in with payment that is another option. Mail it to Phase hockey and your spot is held.
