Phase Hockey programs can be registered for right here on the website. Click Register Now and fill out your information and submit it. Mail your payment in and your spot is held. All your information is kept private. If you wish to fill out an application form by hand and send in with payment that is another option. Mail it to Phase hockey and your spot is held. Its that easy!

PHT makes it easy to sign your child up for any program on-line.Please fill in all the fields below with your information and submit it to Phase Hockey Training. PHT does give this information out to anyone. Once you have completed the on-line form, mail in your payment to Phase Hockey Training. Once your cheque has been deposited you have confirmend and held a spot in any of the great PHT programs you may have chosen. It is that simple!
